First Visit

We at O'Brien Orthodontics are always excited about meeting new patients. The first office visit is complimentary and provides a chance for you to have a thorough examination by Dr. O'Brien and meet our professional staff, who will take diagnostic photos and a 3D x-ray. This visit provides an opportunity to ask questions regarding orthodontic treatment and the entire treatment experience.

Please bring with you the completed medical history form that will be mailed to you prior to your appointment. You may also print the form by clicking on the appropriate link below. If you have orthodontic insurance, please bring your insurance card to the appointment. By providing this information at the first visit, we will be able to inform you about your insurance benefits.
